The short answer

AI generates a likely response from your instructions and context. It does not retrieve one fixed truth each time. Small wording changes, conversation history, model updates, tool access, and built-in variation can produce different answers. Trust the answer you can verify, not the one that sounds most confident.

Stabilize the task

Start a fresh conversation and provide the same question, facts, constraints, date, desired format, and approved sources. Ask the model to separate facts, calculations, assumptions, and recommendations. For current information, require source links and open them yourself.

If two answers differ, make a comparison table:

• Exact disagreement.
• Evidence each answer cites.
• Date and jurisdiction.
• Assumptions.
• Calculation you can reproduce.
• Expert review needed.

Do not ask the model to vote on its own answers. Agreement across several prompts can still repeat the same false assumption.

Match verification to risk

For a social caption, read aloud and check facts. For a price calculation, recompute from source numbers. For software instructions, use the current official documentation and a test account. For legal, tax, medical, financial, safety, privacy, or security decisions, use qualified professionals and authoritative sources.

NIST identifies “confabulation,” confidently stated false or erroneous content, as a generative-AI risk and recommends evaluation and monitoring (NIST Generative AI Profile). Fluent wording is not evidence.

Keep a decision record

For important work, save the prompt, tool and model, date, sources, verified answer, reviewer, and corrections. This helps when outputs change later.

Use AI as a fast junior collaborator: helpful at drafting, organizing, and proposing possibilities, but not the final authority over facts it did not verify. The answer to trust is the one supported by evidence you understand and appropriate human judgment.
